没有多大区别 。an then在用法上比then要严格，不能直接用在名词后，要不就整体放在名词前，要不就拆开在名词前后。

Then：副词，意味着在那个时候，在这种情况下，下一个，或也——“He laughed and thenhe cried.”

I answered two questions and then got stuck.（我回答了两个问题，然后就不懂了。）

I sit up, and through the venetian blinds I can see the palm trees shaking wildly, actually bending, in the hot winds, and then I stare back at the poster and then turn away and then look back again at the smile and the mocking eyes.

then是副词，and是连词，意思上没有区别，但是and then可以引导并列句，而then就不行了。
例：He came here and then he left.这是一个并列句，有两套谓语结构，所以只能用and then作连词。
He came here. Then,he left.这是两个句子，所以可以使用then

个人觉得在意思上是没有区别的，或许在语气上and then更强调前后两个事物时间上联系得更紧密些吧

从口语的角度上说，语义是没有差别的。况且这两个词(组)在写作中不是特别好的词。

then 和 and then, 在口语的使用过程中，完全取决于谈话人的感*彩（情绪）及讲话风格，这是个人掌握的层面。

*The writer is a high school student in England.
